regarding your family member or loved one. A Care Manager
will visit your loved one for an in-home assessment of his/
her situation. This assessment focuses on their strengths and
coping skills, as well as their ability to care for themselves
physically, socially and psychologically. Our assessment
explores how CareLink can add to their quality of life through
referrals to community services, supportive counseling, and the
knowledge that there is someone nearby on whom they may call
at any time.
